Appointment Policies
To cancel or reschedule an appointment, we kindly request that you provide at least 24 hours notice prior to the scheduled appointment time. Otherwise, you will be charged 60$.
If you do not show up for your scheduled appointment without prior notice, you will be charged the full amount for the service.
Please ensure that you have a printed copy of your requisition, as well as a valid Medicare Card Number (valid expiry date) when you arrive for your appointment. A 5$ printing fee will apply if you do not arrive with a printed copy of your requisition.
Water is allowed and encouraged prior to your blood test, even if fasting is required. Staying hydrated can help make the process easier. If your blood test requires fasting, you should not eat or drink anything except water for 8-12 hours prior to your appointment.
Please take the time to contact your insurance provider and confirm if your blood tests are covered under your plan. Often times, your coverage is 100%, don’t miss out!
Doctor's Full Name: The full name of the healthcare provider must be clearly stated.
Doctor's Address: The complete address of the healthcare provider’s office or practice.
Doctor's License Number: The license number of the healthcare provider must be included to validate the requisition.
Fax Number: The healthcare provider’s fax number should be listed for communication purposes.
Doctor's Signature: The requisition form must be signed by the healthcare provider to confirm authorization for the blood test.
Client/patient’s information: Your full name, date of birth and RAMQ number.
It is the client’s responsibility to ensure that the requisition is complete and includes all of the above information before submitting it for processing. Incomplete requisitions may delay your appointment or prevent the test from being conducted.
Please note that we are affiliated with Hawkesbury General Hospital; results are faxed directly to the physician (not available on DSQ). Additionally, we utilize a private laboratory (fees may vary), and those results are accessible through DSQ.
